HCS/TEXAS HOME LIVING (TxHmL)

TxHmL is a Medicaid waiver program approved by the centers for Medicare and Medicaid Services (CMS) to provide community-based services and supports to eligible individuals who live in their own houses or in their family homes. The TxHmL program services only to persons residing in the counties specified in our geographical areas.

• Community Support services are provided to persons residing in all counties within the State of Texas
• Day Habilitation services are provided only to those residing in Harris and Fort Bend Counties.
• Respite services are provided only to those residing in Harris and Fort Bend Counties.

    CASE MANAGEMENT
    Each individual is assigned a Case manager who is responsible for:
    • Coordinating the development and implementation of each person’s Individual Service Plan
    • Coordinating the delivery of each person’s Individual Plan of care
    • Coordinating and monitoring the delivery of waiver and generic services
    • Integrating various aspects of services delivered under the waiver and through other sources
    • Recording each person’s progress
    • Pre-discharge plan development
    • Record keeping
    • Arranging transportation

    RESIDENTIAL ASSISTANCE
    Each individual is given the opportunity of choosing their type of resident8al assistance from the following options:
    • Supported Home Living – this service type is provided to individuals residing in their own residence or the residence of their natural or adoptive family or to individuals receiving foster care services from the Texas Department of Protective and Regulatory Services or the Texas Department of Human Services.
    • HCS Foster/Companion Care – this service type is provided to individuals in a residence by a foster companion care provider exclusive of Texas Department of Protective and Regulatory Services or Texas Department of Human Services foster care programs.
    • Residential Support – This service type is provided to individuals by daily shift staff or by individual staff member with periodic relief staff.

    RESIDENTIAL ASSISTANCE SERVICES INCLUDE:
    • Direct personal assistance with activities of daily living (grooming, eating, bathing, dressing, and personal hygiene)
    • Assistance with meal planning and preparation
    • Securing and providing transportation
    • Assistance with housekeeping
    • Assistance with ambulation and mobility
    • Reinforcement of counseling and therapy activities
    • Assistance with medications and performance of tasks delegated by a Registered Nurse
    • Supervision of individual’s safety and security
    • Facilitating inclusion in community activities, use of natural supports, social interaction, participation in leisure activities, and development of socially valued behaviors
    • Habilitation, exclusive o Day Habilitation

    DENTAL TREATMENT
    Each adult individual as determined by individual needs is eligible for dental services including:
    • Emergency dental treatment
    • Preventive dental treatment
    • Therapeutic dental treatment
    • Orthodontic dental treatment, excluding cosmetic orthodontia

    DAY HABILITATION
    Each adult individual under legal age is provided the opportunity to participate in a day activity, pre-vocational, or vocational setting six (6) hours per day, five (5) days per week as determined by individual needs which provides for:
    • Assisting individuals in acquiring, retaining, and/or improving self-help, socialization, and adaptive skills necessary to reside successfully in the community
    • Providing individuals with age-appropriate activities that enhance self-esteem and maximize functional level
    • Complementing any counseling and therapies listed in the Individual Plan of Care
    • Reinforcing skills or lessons taught in school, therapy, or other settings
    • Training and support activities which promote the individual’s integration in the community
    • Providing assistance for individuals who cannot manage their personal care needs during day habilitation activities.
    • Providing transportation necessary for the individual’s participation in day habilitation activities.
    • Providing transportation necessary for the individual’s participation in day habilitation activities.
    • Providing services which are not funded under Section 110 of the Rehabilitation Act 1973 or Section 602 (16) and (17) of the Individuals with Disabilities Education Act.

    SUPPORTED EMPLOYMENT
    Each individual as determined by individual needs.
    • Employment in an integrated work setting away from the individual’s place of residence
    • Provision of on-going individualized support needed to sustain paid work by the individual, including supervision and training
    • Compensation by the employer to the individual in accordance with the Fair Labor Standards Act.
    • Provisions of services not available or funded through the state education agency or the state rehabilitation agency.

    NURSING
    Each individual as determined by individual needs is provided the services of a nurse to perform health care procedures and monitor health conditions including:

    • Administering medications
    • Monitoring the individual’s use of medications
    • Assisting the individual to secure emergency medical services
    • Making referrals for appropriate medical services
    • Performing health care procedures ordered/prescribed by a Physician/Medical Practitioner
    • Delegation and monitoring of tasks assigned to other service providers by an R.N. in accordance with state law.

    RESPITE
    Each individual living in their family home as determined by individual needs is eligible for respite services to include:
    • Assisting with training in self-help and independent living skills
    • Provision of room and board when provided overnight in a setting other than the person’s normal residence
    • Support for emergency of planned short-term care
    • Support on a 24-hour increment or any part of that increment for no more than 30 days per year
    • Assistance with securing and providing transportation

    ADAPTIVE AIDS
    Each individual as determined by individual needs and authorized by the Interdisciplinary team based on documentation from a licensed specialist as necessary and appropriate to meet the specific needs of the individual is eligible for adaptive aids including:
    • Lifts
    • Mobility aids
    • Control switches/pneumatic switches and devices
    • Environmentally control units
    • Medically necessary supplies
    • Communication aids
    • Repair and maintenance not covered by the warranty

    MINOR HOME MODIFICATION
    Each individual as determined by individual needs as necessary for the health and safety of the individual is eligible for minor home modifications including:
    • Purchase and repair of wheelchair ramps
    • Modification/additions to bathroom facilities
    • Modification/additions to kitchen facilities
    • Specialized accessibility and safety adaptations/additions, including repair and maintenance

    COUNSELING AND THERAPEUTIC SERVICES
    • Audiology services
    • Speech/language pathology services
    • Occupational Therapy services
    • Physical Therapy services
    • Dietary services
    • Social Work services
    • Psychology services
